Disability and Mental Health Support Service

CREATING FUN OPPORTUNITIES, DEVELOPMENT AND SOCIAL SKILLS FOR KIDS, TEENS AND ADULTS

We are super excited to have you join us for our upcoming activity programs. Our goal at Tayla’d Care Services is to provide a supportive, and engaging environment where participants can connect, socialise, learn, grow and have fun.

How to book

To reserve a spot, contact the office via email or telephone or jump on our website and complete the booking form, let us know what activities you or your loved one would like to participate in, and notify us of any accessibility needs or dietary requirements, this needs to be submitted via email or through our website 2-3 weeks prior to activity dates.

Note: Activities and program times may change due to weather, ticket availability, transport, participants and staff attendance. If this happens, you will be notified. If you are booking into an activity that requires a pre-paid ticket or program cost, you are confirming that you have agreed to paying any out-of-pocket costs that have been outlined in your booking confirmation once received. You will be invoiced directly for this cost and will not be required to supply money on the day. If you are a new participant, please contact the office to complete the new participant intake form, so we are able to support and meet your needs correctly. If we can’t reach you your reservation will be cancelled. Payment is required before the activity to secure your spot. Due to popular demand tickets for some of these activities sell out quickly so don’t miss out.

Billing information

NDIS Plan Managed and Self-Managed Participants billed to Capacity Building or Core. Activities are Billed at a 1:2 to 1:3 support ratio, if a different ratio or additional support is required, the cost will be adjusted accordingly.

Out of pocket cost: These are the direct costs participants are responsible for, such as materials, snacks, or activities, that are not covered by NDIS funding. This cost must be pre-paid and will be clearly noted for each activity. NDIS funding costs: These cover the support worker ratio, transport, administration, and centre capital costs (where applicable). These costs are drawn from your NDIS funding, provided they fall withing your NDIS plan’s scope.

Transport

The time set for the activity does not include transport. Participants are to be dropped off and picked up at Ashleigh’s Haven unless transport arrangements have been organised (must be organised prior to the day of the activity). On the expression of interest activity form, you will have the opportunity to select transport. Please note you will be given a time allocation which will be between 15-30 minutes before and after activity. Transport will incur an additional charge.

How to pay

If you are an existing Tayla’d Care Services participant (with a current service agreement), once you book, the support will be added to your roster and included in your weekly invoice. You will not need to pre-pay for NDIS funded support services. If you are not an existing participant, you will be invoiced for the activities you have booked, and pre-payment must be made before the activity date. Please have your service agreements completed no less than 2 weeks prior to the activity commencement date.

Out of pocket costs: Costs such as materials or activity fees, must be pre-paid either via direct transfer or dropping money off at the office.

Please note: These costs are only for the activity participation and materials, and do not cover lunch or personal spending during activities. Prices of programs may change slightly due to online booking fees. If there is an increase in price you will be notified.

Terms and Conditions

  • All attendees are required to abide by Tayla’d Care Services policies and procedures.
  • Payment is required before the activity begins to finalise your booking.
  • Some activities may have restrictions, which will be communicated after booking or upon enquiry.

Cancelation

We understand things can change! As soon as you’re aware you can no longer attend, please get in touch with us: If you become unable to attend a program that you have pre-purchased a ticket for, any refunds will be in line with relevant activity venue cancellation policies.
